Congratulations on taking a step forward in your academic journey. If you’re interested in having me as one of your recommenders, I would need the following from your end via email.

1. Details of what class, section and year I taught you in?
2. Your up-to-date resume.
3. Your up-to-date unofficial transcript.
4. Which other professors have you reached out to for such?
5. Where do you see yourself in five years?
6. What are the top three qualities you would want people to know about your work ethic and personality?
7. Give me your top two to three positions that you're most proud of you where you were a leader and showed leadership skills.
